Purpose• CCPD Competitive Grants Program purpose: Reduce

chronic disease in Colorado by building a cohesive, comprehensive approach that focuses on prevention, early detection, and treatment for cancer, cardiovascular disease, and pulmonary disease

• Amendment 35: voter approved tax increase on cigarettes and other tobacco products – allocates funds to tobacco education, prevention, and cessation programming and prevention, early detection, and treatment of cancer, cardiovascular disease, and pulmonary disease

Purpose (cont.)

• Goals of CCPD Competitive Grants Program: 1. Reduce the number of new cancer cases as well as

illness, disability, and death caused by cancer.2. Reduce cardiovascular disease and related risk

factors.3. Reduce pulmonary disease and related risk factors.

• CCPD also assists in the implementation of the state’s strategic plans for reductions of chronic disease

Funding Availability

• $9 million for the period of October 1, 2012 through June 30, 2013

• Number and amount of grant awards will vary• Renewal funding for FY 14 and FY 15 is

contingent upon grantee performance, assessment based on upcoming Chronic Disease State Plan, and appropriation by Colorado General Assembly

Eligibility

• Eligible organizations: nonprofit orgs, health care orgs, governmental entities, educational institutions, for-profit private sector organizations, federally-recognized Native American Tribes in Colorado

• Applicants encouraged to submit a multi-agency application that designates one agency as lead agency

Exclusions1. Proposals solely focused on research are not appropriate for

this RFA2. Proposals focused on population-based tobacco prevention

and cessation will be considered by a separate RFA related by the Colorado Tobacco Education, Prevention, and Cessation Grants Program and therefore will not be considered by the CCPD Competitive Grants Program.

3. Proposals focused on breast and cervical cancer screening and treatment are solicited by the Women’s Wellness Connection at the Colorado Department of Public Health and Environment (CDPHE) and therefore are not appropriate for this RFA.

4. Building renovations, capital projects, and medical scanning equipment (CT, PET, MRI)

5. Electronic medical records technology that is eligible for incentive payments under the Health Information Technology for Economic and Clinical Health (HITECH) Act

6. Internal and external defibrillators (AEDs)7. Programs that supplant existing funding for services 8. Research projects9. Spirometry screening for asymptomatic smokers10. Prostate screening11. Screening programs that do not link people to diagnostic

services

Funding Areas/Requirements

• CCPD Review Committee adopted a funding framework that aligns with the Health Impact Pyramid, specifically focusing on interventions with the greatest impact such as interventions that change the context to make individuals’ default decisions healthy, clinical interventions that require limited contact but confer long-term protection, and ongoing direct clinical care

• CCPD Competitive Grants Program will prioritize comprehensive approaches for chronic disease prevention and risk factor reduction and seeks to concentrate resources in the following public health domains:

1. Evidence-based practices and environmental change: population-based interventions that transform communities to support health and make healthy behaviors easier and more convenient for people

2. Health systems change: interventions that improve the use of quality clinical services to prevent disease, detect disease early, and manage risk factors

3. Community-clinical linkages: interventions that ensure those with and at-risk for chronic disease have access to quality community resources to best manage their conditions and prevent disease onset

4. Surveillance and epidemiology: activities to gather, analyze, interpret, and disseminate data and conclusions about disease (Please note: applicants of this RFA will not apply for these activities.)

Recipient Activities

• RFA solicits applications from eligible entities to apply for competitive funding for year one of a three-year grant cycle

• Applicants encouraged to collaborate with other agencies and submit a multi-agency application that designates one agency as lead agency

• Coordination of services and collaboration with LPHAs in proposed project is expected

Recipient Activities (cont.)• Grants applications shall address at least one of

following program criteria (described in statute)– Translating evidence-based strategies into practical

application in health care, workplace, and community settings

– Providing appropriate diagnosis and treatment services for anyone who has abnormalities discovered in screening and early detection programs

– Implementing education programs regarding prevention, early detection, and treatment of CCPD

– Providing evidence-based strategies to overcome health disparities in prevention and early detection of CCPD

categories:1. Community-level approaches for chronic

disease prevention2. Statewide approaches for chronic disease

• Proposed strategies and activities are to meet one or more of CCPD Competitive Grants Program goals

public health domains and propose strategies that meet one or more of following domains to implement a comprehensive approach for chronic disease prevention:

Recipient Activities (cont.)• Community-level approaches for chronic

disease prevention: intent is to implement evidence-based practices and environmental changes that comprehensively prevent chronic disease within a selected geographic area in Colorado– Applicants for community-level approaches that are

not LPHAs are required to obtain letters of collaboration/support from the LPHA located within the targeted geographic area(s) specified in the proposal

Application Requirements

• Applicants will compete against each other for grant awards

• CCPD Review Committee reserves the right to negotiate budgets and work plans with selected eligible applicants depending on the number of eligible applicants, which many impact award start date

Application Review Process & Evaluation Criteria

• Applications will be scored on a 100-point scale:– Needs Statement (10 points)– Project Narrative (30 points)– Work Plan (30 points)– Evaluation (10 points)– Budget (20 points)

• Minimum score of 70 points is required to be considered for funding

• CCPD Review Committee will convene in late July 23rd and 24th to arrive at final funding recommendations

• CCPD Review Committee will present award recommendations to Colorado Board of Health for final approval on Wednesday, August 15, 2012– Award decisions by the Board of Health are not

subject to appeal
