particularly important for facial development. Distur-bances during embryogenesis in this area may lead toabnormal development of the lower midface, ear,mandible, and neck [8]. In some cases, derivatives of thesecond branchial arch are involved additionally, causingsymptoms in the ocular and cervical spine regions. Prema-ture involution of branchial arch arteries is thought to bethe etiologic pathogen [16]. The earlier the ischemic invo-lution appears, the more severe is the resulting dysplasia.Classification of these malformations follows the involvedbranchial arches. Unilateral forms include Goldenhar syn-drome, and bilateral forms Franceschetti syndrome andNager syndrome.

Goldenhar SyndromeThere are two different manifestations of this unilateralmandibulofacial dysplasia, which had been assumed in theearlier literature to be distinct entities: hemifacial micro-somia or otomandibular dystosis as a mild expression, andoculo-auriculovertebral dysplasia or Goldenhar syndromeas a severe phenotype [5]. In the more recent literature,however, this distinction is rejected in view of transitionalforms and the sporadic simultaneous occurrence of bothvariants in relatives [19]. The etiology is unknown andprobably heterogeneous. Autosomal dominant as well asautosomal recessive inheritance has been reported in somecases [15]. About two thirds of all affected patients aremale.

Hemifacial microsomia is a congenital defect involv-ing primarily the first branchial arch derivatives. Resultsfrom animal experiments suggest disruption of a branchialarch artery during early embryogenesis as the underlyingpathogenic factor [16, 21]. The phenotype is highly vari-able. The generally strictly unilateral symptoms encom-pass a facial asymmetry resulting from unilateral facialhypoplasia with atrophy of the masseter and temporalmuscles, hemimandibular hypoplasia, cleft lip, alveolus,and palate, macrostomia, auricular hypoplasia with orwithout reduction in size, pre-auricular fistulae orappendages, and similar features between ear, corner ofthe mouth, and neck [6]. Conductive hearing loss may arisefrom the additional dysgenesis of middle ear ossicles. Rareintercurrent symptoms are semilingual hypoplasia, aplasiaof one parotid gland, and malformations of the pyramid.

Oculo-auriculovertebral dysplasia affects the first andsecond branchial arches and is accompanied by oph-thalmic symptoms with epibulbar dermoid cysts of the low-er lid and lipodermoid tumors of the upper lid, both occur-ring laterally in most cases, upper lid coloboma,strabismus, and microphthalmia. In rare cases anoph-thalmia, coloboma of the iris and retina, and antimon-goloid angulation of the palpebral fissures are observed.Further symptoms are defects of the upper cervical spine,such as spina bifida, hemivertebrae, and spinal fusion as

Als zugrunde liegende Morphogenesestörung wird einevorzeitige Involution der Viszeralbogenarterien diskutiert[16]. Je früher die ischämiebedingte Involution einsetzt,umso höher ist der Grad der daraus resultierenden Fehlbil-dung. Eine Einteilung dieser Fehlbildungen orientiert sichan den entsprechenden Viszeralbögen. Typische Krankheits-bilder bei einseitigem Auftreten sind das Goldenhar-Syn-drom und bei beidseitigem Auftreten das Franceschetti-Syn-drom sowie das Nager-Syndrom.

Goldenhar-SymptomenkomplexDiese einseitige mandibulofaziale Dysostose kommt inzwei verschiedenen Varianten vor, die in der älteren Litera-tur noch als zwei getrennte Syndrome aufgefasst wurden,nämlich als hemifaziale Mikrosomie oder Dysostosis oto-mandibularis (leichtere Form) und als okulo-aurikulo-ver-tebrale Dysplasie oder Goldenhar-Syndrom (schwerereForm) [5]. In der neueren Literatur wird jedoch aufgrundvon Übergangsformen und des gelegentlich gemeinsamenfamiliären Vorkommens diese Trennung nicht mehr vorge-nommen [19]. Die Ätiologie ist unbekannt und vermutlichheterogen. Vereinzelt wurde ein autosomal-dominanterund auch autosomal-rezessiver Erbgang beschrieben [15].Etwa zwei Drittel der betroffenen Patienten sind männlich.

Die hemifaziale Mikrosomie ist eine angeborene Fehl-bildung, die vor allem die Derivate des ersten Viszeralbo-gens betrifft. Aufgrund von Tierversuchen könnte eine vas-kuläre Disruption einer Halsarterie in der frühen Embryo-genese pathognomonisch sein [16, 21]. Der Phänotyp ist sehrvariabel. Die meist streng unilateralen Befunde umfassen ei-ne Gesichtsasymmetrie infolge einer einseitigen Gesichtshy-poplasie mit verminderter Muskelmasse des Musculus mas-seter und temporalis, eine Hypoplasie einer Unterkieferhälf-te, eine Lippen-Kiefer-Gaumenspalte, ein Makrostomasowie eine Hypoplasie der Ohrmuschel mit oder ohne Re-duktion, präaurikulären Fisteln oder Anhängseln und ähnli-chen Fehlbildungen auf einer Linie vom Ohr zum Mundwin-kel und Hals [6]. Schallleitungsstörungen können bei zusätz-lichen Fehlbildungen der Mittelohrknochen auftreten.Weitere seltene Fehlbildungen sind eine einseitige Zungen-hypoplasie, ein Fehlen der Glandula parotidea und Fehlbil-dungen der Pyramide.

Bei der okulo-aurikulo-vertebralen Dysplasie sind dererste und zweite Viszeralbogen betroffen und kommenaußerdem noch Symptome am Auge mit einem meist latera-len epibulbären Dermoid am Unterlid, einem Lipodermoidmeist lateral am Oberlid, einem Oberlidkolobom, Strabis-

well as thoracic and abdominal anomalies. Mental retarda-tion is reported in 15% of affected patients [19].

Case 1 (born in 1968) Symptoms

TreatmentSurgical cleft repair was performed at the age of 7 months.During the primary and mixed dentition period effortswere made, using various functional appliances, to controlthe asymmetric facial development resulting from right-sided hypoplasia of the mandibular hard and soft tissue(Figure 1b). The dental germs of the secondary dentitionwere preformed without exception, but all four first pre-molars had to be extracted due to pronounced dentalcrowding. The dental arches were aligned by means ofremovable appliances (fixed appliances were not yet at ourdisposal at that time). Orthodontic treatment was tem-porarily abandoned when the patient was 13 years old(Figure 1c). Maxillofacial surgery was subsequently per-formed on the lip, nose, and chin in combination withmandibular and auricular augmentation. At the age of 26 years, a bimaxillary orthognathic osteotomy3 withmandibular reconstruction for further improvement of thefacial symmetry was performed following realignment ofthe upper and lower dental arches for an 11-month period,using fixed appliances (Figure 1d). The final outcome isshown in Figure 1e. Figure 1f shows the improved facialsymmetry in longitudinal view. Table 1 shows the timeschedule for interdisciplinary orthodontic therapy andmaxillofacial surgery.

Case 2 (born in 1975) Symptoms

Hypoplasia of the right facial hard and soft tissue includingthe mandibular ramus, auricular malformation, completecleft of the lip, alveolus, and palate, horizontal and obliquefacial clefts, macrostomia, eyelid coloboma, corneal opaci-fication, and entropion of the lower lid. Figure 2a shows

Behandlungsverlauf Der operative Lippenverschluss wurde im Alter von 7 Mo-naten durchgeführt. Mit wechselnden Aktivatoren wurdeim Milch- und Wechselgebissalter versucht, die asymmetri-sche Gesichtsentwicklung infolge der rechtsseitigen Unter-kieferknochen- und umgebenden Weichteilhypoplasie auf-zuhalten (Abbildung 1b). Alle bleibenden Zähne warenangelegt. Wegen ausgeprägter Engstände erfolgte die syste-matische Extraktion aller ersten Prämolaren und an-schließend eine weitere Ausformung beider Zahnbögen mitherausnehmbaren Apparaturen. (Festsitzende Apparatu-ren standen zu diesem Zeitpunkt in unserer Einrichtungnoch nicht zur Verfügung.) Die kieferorthopädische Be-handlung war im Alter von 13 Jahren zu einem vorläufigenAbschluss geführt worden (Abbildung 1c). Es folgten kie-ferchirurgische Korrekturmaßnahmen an Lippe, Nase,Kinn sowie Aufbauplastiken an Unterkiefer und Ohren. ImAlter von 26 Jahren wurde eine bimaxilläre Dysgnathiekor-rektur mit Rekonstruktion des Unterkiefers zur weiterenVerbesserung der Gesichtssymmetrie ausgeführt3, nachdemvorbereitend über einen Zeitraum von 11 Monaten einenochmalige Ausformung der Zahnbögen im Ober- und Un-terkiefer mit Multibracket-Apparaturen stattgefundenhatte (Abbildung 1d). Das Endergebnis ist in Abbildung 1ezu sehen. Abbildung 1f zeigt im Längsschnitt die Verbesse-rungen der fazialen Asymmetrie. Tabelle 1 gibt die kiefer-chirurgischen und die kieferorthopädischen Behandlungs-schritte in der zeitlichen Folge wieder.

the course of the primary dentition period. Various remov-able und functional appliances were used for orthodontictherapy during the mixed dentition period. Both lowerfirst premolars were extracted to counterbalance the apla-sia of the two upper lateral incisors. The pronounced labi-al and lingual dysfunction proved to be exceptionallyproblematic and largely resistant to treatment despiteintensive speech therapy. Certainly, this dysfunction wasdue at least in part to the congenitally disturbed lingualinnervation and led to the development and progression ofan open bite (Figures 2b and 2c). Eventually, at the age of16 years, the treatment was temporarily abandoned afterconsultation with the patient and his parents, with the aimof solving the problems by means of orthognathic surgeryand adjuvant orthodontic therapy after completion ofcraniofacial growth. Four years later, orthodontic treat-ment was resumed using fixed appliances for preoperativealignment of the dental arches (Figure 2d). Lengthening ofthe mandible by gradual bone distraction after osteotomyon the right side, using an extraoral device, was followedby a bimaxillary orthognathic osteotomy and tertiary alve-olar bone grafting 7 months later. Postoperative orthodon-tic adjustment using fixed appliances provided furtherocclusal improvement. Surgical nasal correction and therequired definitive prosthodontic treatment completed therehabilitation measures, apart from otoplasty which is stillpending (Figure 2e). Figure 2f illustrates the craniofacialimprovements step by step. The steps involved in the max-illofacial surgery and orthodontic therapy are listed inchronological order in Table 2.

Franceschetti or Treacher Collins SyndromeMandibulofacial dysostosis, known as Treacher Collins orFranceschetti syndrome, is an autosomal dominantmandibulofacial disorder with complete penetrance andvariable expression [1, 3, 4, 18]. In sporadic cases the dif-

BehandlungsablaufNach den primären Verschlussoperationen von Lippe undGaumen schlossen sich im Milchgebissalter die Makrosto-makorrektur und eine Velopharyngoplastik an. Die kiefer-orthopädische Behandlung im Wechselgebissalter war mitaktiven Platten und verschiedenen funktionskieferorthopä-dischen Apparaturen durchgeführt worden. 34 und 44 wur-den wegen Nichtanlage von 12 und 22 gegenextrahiert. Alsbesonders problematisch erwies sich eine ausgeprägte Lip-pen- und Zungendysfunktion, die trotz intensiver sprech-therapeutischer Behandlung nicht ausreichend korrigiertwerden konnte. Sie war sicherlich teilweise auch durch dieangeborene nervale Innervationsstörung der Zunge mitbe-dingt. Das Auftreten und die Verstärkung des dadurch be-dingten offenen Bisses ließ sich nicht verhindern (Abbil-dungen 2b und 2c). Schließlich kam es in Absprache mitden Eltern und dem Patienten im Alter von 16 Jahren zueiner Unterbrechung der Behandlung mit dem Ziel, nachAbschluss des Wachstums die Umformung der Zahnbögenfortzusetzen und anschließend die Dysgnathie kieferchirur-gisch zu korrigieren. Die prächirurgische kieferorthopädi-sche Behandlung mit festsitzenden Apparaturen wurde mit20 Jahren eingeleitet (Abbildung 2d). Es folgte eine rechts-seitige Distraktionsosteogenese mit einer extraoralen Di-straktionsapparatur, 7 Monate später eine bimaxilläre Dys-gnathiekorrektur und eine Alveolarspaltosteoplastik. Einepostchirurgische Justierung mit Multibandapparaturen ver-besserte weiter die Okklusion. Eine operative Nasenkor-rektur und die erforderliche definitive prothetische Versor-gung schlossen die Rehabilitationsmaßnahmen – bis auf dienoch ausstehende Ohraufbauplastik – ab (Abbildung 2e).

ferential diagnosis is problematic due to the pronouncedvariance of expressivity. Affected females more often haveaffected descendants than affected males. The underlyinggenetic locus maps to the long arm of chromosome 5 at5q32-33.1 [2]. About 60% of cases are caused by novelmutations. The pathogenesis is not yet fully clarified. Thelikelihood is that alteration of the intercellular matriximpedes the cellular spread during morphogenesis in thearea of the first branchial arch and the first branchialpouch [9]. In the mouse animal model, hypervitaminosis Agenerates a similar teratism [9].

Patients display antimongoloid angulation of thepalpebral fissures, sharp bends within the lateral halves ofthe lower eyelids, irregularly marked lower lid coloboma(in contrast to Goldenhar syndrome), and missing tarso-conjunctival glands and cilia [18]. The frontonasal angle is

Franceschetti- oder Treacher-Collins-SyndromBei der Dysostosis mandibulofacialis, dem Franceschetti-oder Treacher-Collins-Syndrom, handelt es sich um ein autosomal-dominant vererbtes, mandibulofaziales Dysmor-phiesyndrom mit vollständiger Penetranz und variabler Ex-pressivität [1, 3, 4, 18]. Die starken Ausprägungsschwan-kungen bereiten bei sporadischen Fällen gelegentlich diffe-renzialdiagnostische Schwierigkeiten. Betroffene Frauenhaben mehr betroffene Nachkommen als Männer. Das ver-antwortliche Gen ist auf dem langen Arm von Chromosom5 lokalisiert (5q32-33.1) [2]. Etwa 60% der Patienten sindNeumutanten. Die Pathogenese ist nicht widerspruchsfreigeklärt. Wahrscheinlich kommt es durch Alteration der in-terzellulären Matrix zu einer Behinderung der morphoge-netischen Zellwanderungen im Entwicklungsfeld des erstenViszeralbogens und der ersten Viszeralfurche [9]. Eine Hypervitaminose A erzeugt bei der Maus ein ähnlichesFehlbildungssyndrom [9].

Die Patienten zeigen einen antimongoloiden Lidspal-tenverlauf, eine Abknickung des Unterlidrandes in deräußeren Hälfte mit mehr oder minder deutlichem Unterlid-kolobom (im Gegensatz zum Goldenhar-Symptomenkom-plex) sowie ein Fehlen der Meibom-Drüsen und der Wim-pern [18]. Der Stirn-Nasen-Winkel ist flach mit zu groß er-scheinender Nase. Weiterhin bestehen eine Hypo- oderAplasie des Jochbeinbogens, eine Hypoplasie der Maxillamit kleinen Kieferhöhlen und dadurch eingesunkene Wan-gen, verschieden schwere Reduktionsfehlbildungen desäußeren Ohres mit oder ohne Gehörgangsstenose oder -atresie und dadurch bedingter Schallleitungsstörung bisTaubheit sowie eine Makro- oder auch Mikrostomie mit

flattened and the nose projecting. Further symptoms arehypoplastic or even aplastic zygomatic arches, maxillaryhypoplasia with reduced sinuses and resulting hollowcheeks, inconsistently severe auricular dysplasia with orwithout stenosis or atresia of the external auditory meatusand concomitant conductive hearing loss or deafness,macrostomia or alternatively microstomia with a narrowand high palatal vault, and a cleft palate in about 35% ofcases. Dental malalignments with anterior open bite arefrequent. The hypoplastic mandible makes the chin seemsmall and markedly receding, and the lower mandibularmargin concave [13]. Fistulae and/or dermal appendagesoccur between the ears and labial angles, and abnormaltemporal and buccal hypertrichiosis is also reported.

Case 3 (born in 1973)This patient was referred to our department at the age of15 years prior to entering a boarding college for pupils withimpaired hearing.

Symptoms Mandibular retro- and micrognathia, enlarged overjet,reduced mouth opening, deviation of the dental midlinefollowing extraction of the left lower first molar, and a sin-istroinferiorly sloping occlusal plane. Craniofacial featureswere hypoplastic zygomatic arches, antimongoloid angula-tion of the palpebral fissures, receding chin, low-sited ears,and atresia of the external auditory meatus (Figure 3a).

TreatmentExercises in mouth opening and mandibular protrusion.Application of a functional appliance for mandibularadvancement and protrusion had proved to be only mod-erately effective. A modified treatment schedule wasestablished with the therapeutic measures performed inthe following sequence: preoperative alignment of themaxillary and mandibular dental arches, extraction of allthird molars, mandibular advancement by sagittal osteoto-my of the ramus, bilateral zygomatic augmentation, genio-plasty, rhinoplasty, and postoperative adjustment of the

Kasuistik 3 (geb. 1973)Der damals 15-jährige Patient war wegen des Besuchseines Internat-Gymnasiums für Schwerhörige von auswärtsüberwiesen worden.

SymptomatikMandibuläre Retro- und Mikrognathie, große sagittaleSchneidekantenstufe, eingeschränkte Mundöffnung, al-veoläre Mittellinienverschiebung durch eine vorausgegan-gene Extraktion von 36 und eine nach links unten gekippteOkklusionsebene. Faziale Merkmale waren Jochbogenhy-poplasie, antimongoloider Lidachsenverlauf, fliehendesKinn, tiefsitzende Ohren und Gehörgangsatresie (Abbil-dung 3a).

BehandlungsablaufMundöffnungs- und Unterkiefervorschubübungen. DasTragen eines Funktionsreglers zur Vororientierung undNachentwicklung des Unterkiefers als bisherige apparativeTherapie hatte nur einen mäßigen Erfolg gezeigt. Ein er-neut aufgestellter Behandlungsplan sah folgende Maßnah-men vor, die auch in dieser Reihenfolge abgearbeitet wur-den: prächirurgische Ausformung des Ober- und Unterkie-ferzahnbogens, Extraktion der Weisheitszähne, Unterkie-fer-Vorverlagerung durch sagittale Osteotomie, beidseitigeJochbogen-Aufbauplastik, Genioplastik, Nasenkorrektur,postchirurgische Feinkorrektur der Okklusion. Das Be-handlungsergebnis ist in Abbildung 3b dargestellt. Diebrückenprothetische Versorgung der Lücke bei 36 sollte imHeimatort erfolgen. Abbildung 3c zeigt den Profilvergleichmit 15 und mit 20 Jahren. Tabelle 3 listet wiederum den kie-

occlusion. The final outcome is shown in Figure 3b.Prosthodontic treatment concerning the missing lower leftfirst molar is scheduled by the family dentist. Figure 3cmakes it possible to compare the facial profile at the age of15 and 20 years. Table 3 summarizes the maxillofacial andorthodontic treatment course in chronological order.

Nager SyndromeAcrofacial preaxial dysostosis, known as Nager syndrome,is a manifestation of a field defect due to a disturbed blas-togenesis during the 3rd and 4th weeks of embryogenesis,which ultimately involves different structures. There aredifferent opinions on the etiology and the contribution ofgenetic factors. Most cases seem to be spontaneous. Inter-mittent hereditary occurrence suggests an irregularly auto-somal dominant or recessive inheritance [14, 15].

Facial symptoms resemble Franceschetti syndrome com-bined with hypoplastic radial limb deformities. Hypoplasia orabsence of the thumbs are diacritical features [12]. Furthersymptoms are radio-ulnar synostosis, hypoplasia of the radius,and costal as well as vertebral malformations. In rare cases,additional tibial limb anomalies were observed [17].

Case 4 (born in 1970)Symptoms

Mandibular retro- and micrognathia, microglossia, glos-soptosis, cleft and hypoplastic velum, laryngeal hypopla-sia, absence of the right and hypoplasia of the left thumb,which was removed at the age of 2 years due to defunc-tionalization (Figures see [14]).

TreatmentMandibular and laryngeal hypoplasia as well as glossoptosiscaused airway obstruction and feeding problems in the new-born child, necessitating long-term care on a neonatal ward.However, these problems were successfully controlled withconservative measures such as nasogastric tube feeding andresting the child in prone position or on either side.

The planned primary velopharyngoplasty was sched-uled for a later date from the very outset due to retardationof the physical development of the child, velopharyngealinsufficiency, and limited mouth opening. However, the pri-mary velopharyngoplasty attempted at the age of 4, 14 andnearly 18 years failed because endotracheal intubationproved to be impossible. As expected, the velopharyngealfunction was insufficient, since the velum remainedhypoplastic and unmoving due to the cleft velar muscles, andspeech was accordingly inadequate despite speech therapy.

During the primary and mixed dentition period,orthodontic treatment with a functional appliance provedto be generally ineffective (Figures 4a and 4b). However,the severely limited mouth opening was substantiallyimproved through regular mouth-opening exercises usingan increasing number of interocclusal wooden spatulas.

Nager-SyndromDie akrofaziale-präaxiale Dysostose oder das Nager-Syn-drom wird als Ausdruck eines mehrere Strukturen betref-fenden Felddefektes betrachtet, der auf Störungen der Bla-stogenese während der 3. und 4. Embryonalwoche zurück-zuführen ist. Über die Ätiologie und die Beteiligung gene-tischer Faktoren existieren unterschiedliche Ansichten.Sporadisches Vorkommen ist am häufigsten; bei familiäremVorkommen wird ein unregelmäßig autosomal-dominanteroder rezessiver Erbgang angenommen [14, 15].

Die Gesichtsbefunde gleichen denen des Franceschetti-Syndroms, kombiniert mit Reduktionsfehlbildungen des ra-dialen Strahls der Extremitäten. Hypoplasie oder Aplasieder Daumen sind obligat [12]. Daneben werden eine radio-ulnare Synostose, eine Hypoplasie des Radius und Rippen-und Wirbelanomalien beobachtet. In seltenen Fällen istauch der tibiale Strahl der unteren Extremitäten betroffen[17].

Kasuistik 4 (geb. 1970)Symptomatik

Mandibuläre Retro- und Mikrognathie, kleine zurücklie-gende Zunge, Velumspalte mit Hypoplasie der gespaltenenVelumanteile, laryngeale Hypoplasie, Aplasie des rechtenund Hypoplasie des linken Daumens, der wegen Funk-tionsuntüchtigkeit im Alter von 2 Jahren entfernt wurde(Abbildungen s. [14]).

BehandlungsablaufDie mandibuläre und laryngeale Hypoplasie sowie die re-troplatzierte Zunge führten bei dem Neugeborenen zuLuftwegsobstruktionen und Ernährungsproblemen, dieeinen längeren stationären Aufenthalt nach sich zogen,aber mit konservativen Maßnahmen (Sondenernährung,Bauchlagerung bzw. wechselnde Seitenlagerung des Neuge-borenen) beherrscht werden konnten.

Die vorgesehene primäre Velopharyngoplastik wurdewegen der Verzögerung in der körperlichen Entwicklung desKindes, wegen der Insuffizienz des Velums und wegen derreduzierten Mundöffnung von vornherein später geplant.Die im Alter von fast 4, 14 und fast 18 Jahren vorgenomme-nen Operationsversuche scheiterten jedoch an der Unmög-lichkeit der endotrachealen Intubation. Erwartungsgemäßwar auch wegen der Kürze und der geringen Beweglichkeitdes Velums aufgrund der gespaltenen Velummuskulatur dervelopharyngeale Abschluss nicht gegeben und die Sprachetrotz Übungsbehandlung entsprechend schlecht.

Die kieferorthopädische Behandlung mit einem funkti-onskieferorthopädischen Gerät in der Milch- und Wechselge-bissphase war wenig erfolgreich (Abbildungen 4a und 4b). Eskonnte aber durch regelmäßige Dehnungsübungen mit stei-

All permanent teeth were visible in the radiographs.Owing to the bimaxillary hypoplasia there was a seriouslack of space within the dental arches, so that all four firstpremolars had to be extracted in the course of orthodontictherapy. After an interval of several years, orthodontictreatment was resumed at the age of 18 years, with bothdental arches being aligned in preparation for surgicaladvancement of the mandible.

At the age of 19, sagittal splitting of the mandibularramus and protraction according to Obwegeser and DalPont was performed together with chin augmentation.However, the mandibular retrognathia could not be com-pletely corrected, and a partial relapse was recorded in thefurther course, probably due at least in part to inadequate-ly developed soft tissue structures and a hypoplastic tem-

Nacheiner mehrjährigen Pause wurde im Alter von 18 Jahren einekieferorthopädische Behandlung zur Ausformung der Einzel-kiefer als vorbereitende Maßnahme für eine operative Vorver-lagerung des Unterkiefers wieder aufgenommen.

Mit 19 Jahren erfolgte die Umstellungsosteotomie des Unterkiefers und eine Kinnaufbauplastik. Die mandibuläreRetrognathie konnte nicht vollständig beseitigt werden. Zu-sätzlich trat im weiteren Verlauf ein Teilrezidiv auf, das zu-mindest teilweise auf die mangelhaft ausgebildeten Weich-

poromandibular joint. Supraforaminal osteotomy of theramus and bilateral application of a distraction devicewere therefore performed one year later (Figure 4c). Theadvancement achieved was striking and remained stableeven after removal of the bimaxillary positioner as retain-ing appliance. The facial appearance was considerablyimproved. Unfortunately, both lower first molars provedto have ankylosed during the postoperative orthodontictreatment, resulting in imperfect occlusion in these regions(Figures see [14]). Changes in the facial profile duringtreatment are shown in Figure 4d. The individual stepsinvolved in the treatment are given in chronologicalsequence in Table 4.

DiscussionIn general, previously published case reports describetreatment of such patients only within a restricted periodof either infancy or adulthood. The courses of the veryresource-intensive therapy and the final outcomes pre-sented here are aimed at recording the oral and maxillofa-cial surgery and the orthodontic measures applied frombirth to adulthood and at considering how treatment maybe optimized. A hereditary factor had been detected innone of these patients; each of the congenital malforma-tions had occurred spontaneously. Good interdisciplinarycoordination of treatment limited to the correction of thephenotype is very important in these cases. The periodrequired for therapy takes from birth to adulthood. Itmust, however, be borne in mind that all patients referredto in the present study had been born around 1970. In themeantime, technically improved endoscopes are availablefor endotracheal intubation during the anesthesia of suchpatients. Therefore, intubation failures, as presented incase report 4, are unlikely to occur today.

The use of functional appliances during the primaryand mixed dentition was aimed at stimulating mandibulargrowth and preventing further asymmetric development incases of unilateral dysplasia. In accordance with the state

Die erreich-te Vorverlagerung war erheblich und konnte auch nach Ab-setzen des Positioners zur Retention stabil gehalten werden.Das faziale Erscheinungsbild hatte sich deutlich verbessert.Bedauerlicherweise stellte sich während der postchirurgi-schen kieferorthopädischen Behandlung heraus, dass 36 und46 ankylosiert waren, wodurch in diesen Regionen keine zu-friedenstellende Okklusion zustande kam (Abbildungen s.[14]). Abbildung 4d zeigt die Veränderungen im Profilver-lauf bis zum Abschluss der Behandlung. Tabelle 4 beschreibtwiederum die einzelnen Behandlungsschritte in der zeitli-chen Abfolge.

DiskussionBisher sind ausschließlich Kasuistiken publiziert worden,die die Behandlung nur in einem eingeschränkten Zeitraumwährend des Kindesalters oder bei Erwachsenen darstell-ten. Die hier gezeigten Verläufe der sehr aufwendigen The-rapie und die Darstellung der Endzustände sollen den Ein-satz kieferchirurgisch/kieferorthopädischer Maßnahmenvon der Geburt bis in das Erwachsenenalter belegen undÜberlegungen einschließen, wie die Behandlung effektivergestaltet werden kann. Familiäres Vorkommen war bei kei-nem der hier gezeigten Patienten nachgewiesen worden; dieangeborene Fehlbildung war jeweils spontan aufgetreten.Die ausschließlich symptomatische Möglichkeit der Be-handlung dieser Patienten muss interdisziplinär gut abge-stimmt erfolgen. Der Zeitraum dafür erstreckt sich von derGeburt bis in das Erwachsenenalter. Bei den gezeigten Pati-enten muss aber berücksichtigt werden, dass sie um 1970 ge-boren worden sind. Inzwischen stehen technisch wesentlichverbesserte Endoskope für eine Intubation bei der Narkosesolcher Patienten zur Verfügung. Deshalb kann davon aus-gegangen werden, dass heute erfolglose Intubationsversu-che wie bei Patient 4 nicht zu erwarten sind.

of the art at that time, such appliances were used to bridgethe period until the patient was old enough for the neces-sary mandibular surgery. The therapeutic efficacy of thistreatment is difficult to assess. Although no visible successwas recorded, a wait-and-see attitude might have rein-forced the skeletal discrepancies.

Meanwhile, osteogenesis by gradual distraction repre-sents an established and appropriate procedure for length-ening the hypoplastic mandible resulting from these severecongenital skeletal maldevelopments. This method can beused in early childhood in order to adapt the size and posi-tion of the mandible earlier and more efficiently [7, 11, 20,22]. One of its most important advantages is the gradualdistraction and the simultaneous distension of the peri-mandibular soft tissues which are also affected by the con-genital dysplasia. This reduces the risk of relapse in com-parison with conventional methods of displacementosteotomy for skeletal correction. Surgical advancementof the mandible by sagittal splitting of the ramus mightpossibly have been avoided in the present patients if theprocedure of gradual mandibulofacial bone lengtheninghad been at our disposal in the 1970s. However, orthodon-tic treatment with functional appliances would still havebeen necessary to stabilize the postoperative result.

After the introduction of gradual bone lengthening tooral and maxillofacial surgery, this method was applied topatients 2 and 4 in adulthood, in patient 2 before and inpatient 4 after orthognathic surgery. In patient 2, distrac-tion was performed unilaterally to lengthen the atrophichemimandible to improve preconditions for the bimaxil-lary orthognathic surgery needed to correct skeletal dis-crepancies. In patient 4, bilateral gradual bone lengtheningwas carried out successfully after partial relapse followingorthognathic surgery. A marked and stable adaptation ofthe size of the mandible to that of the maxilla with conse-quent improvement of the facial profile was achieved.

Regarding distraction osteogenesis, the function ofthe orthodontist in cooperation with the oral and maxillo-facial surgeon consists in interdisciplinary, very precisetreatment planning, preoperative alignment of the dentalarches, and prevention or at least reduction of deviationsof the occlusal plane in good time through targetedencouragement of vertical growth and elimination of den-tal compensation and crowding in the dental arch. Ortho-dontic treatment is performed with acrylic splints or func-tional appliances with selected bite planes that undergoindividual occlusal adjustment to achieve selective extru-sion of the molars and premolars on the affected side andto regulate the transversal position of the mandible, espe-cially in cases of unilateral dysplasia.

Distraction osteogenesis demands intensive, often long-term orthodontic aftercare to achieve correct alignment ofthe teeth and controlled closure of lateral open bite. Simul-taneous or subsequent treatment with intermaxillary elastics

Nach dem damali-gen Stand des Wissens sollte mit dem Tragen dieser Appara-turen auch die Zeit überbrückt werden, bis der Patient dasAlter für die notwendige kieferverlagernde Operation er-reicht hatte. Die positive Wirkung dieser Behandlung istschwer einzuschätzen. Sichtbare Erfolge waren nicht zu er-kennen, aber eine nur abwartende Haltung hätte möglicher-weise die skelettalen Diskrepanzen noch verstärkt.

Inzwischen ist die Distraktionsosteogenese als Verfah-ren zur Verlängerung des hypoplastischen Unterkiefers beidiesen ausgeprägten angeborenen skelettalen Wachstums-abweichungen anerkannt und geeignet, bereits im Kindesal-ter angewendet zu werden, um die Anpassung des Unterkie-fers in Größe und Lage frühzeitiger und effektiver zu errei-chen [7, 11, 20, 22]. Einer ihrer größten Vorteile ist dieschrittweise Distraktion und die gleichzeitige Dehnung derden Knochen umgebenden Weichgewebe, die ebenfalls vonder angeborenen Störung betroffen sind. Dadurch ist die Re-zidivgefahr geringer als bei dem herkömmlichen Verfahrender Umstellungsosteotomie zur skelettalen Korrektur. Mög-licherweise hätte bei unseren Patienten auf eine chirurgischeUnterkiefervorverlagerung durch sagittale Unterkiefer-osteotomie im Erwachsenenalter verzichtet werden können,wenn die Methode der Distraktionsosteogenese im Kiefer-Gesichts-Bereich bereits in den 1970er Jahren zur Verfügunggestanden hätte. Eine Nachbehandlung mit funktionskiefer-orthopädischen Apparaturen wäre aber auch dann noch er-forderlich gewesen, um das Ergebnis zu stabilisieren.

Nach Einführung in die Kieferchirurgie war die Distrak-tionsosteogenese bei den Patienten 2 und 4 im Erwachse-nenalter angewendet worden, und zwar einmal vor und ein-mal nach der Umstellungsosteotomie. Im Fall von Patient 2war die einseitige Distraktion zur Verlängerung der von derFehlbildung betroffenen Unterkieferseite eingesetzt wor-den, um bessere Voraussetzungen für eine bimaxilläre Dys-gnathieoperation zu erreichen, die wegen der skelettalenDiskrepanzen erforderlich war. Im Fall von Patient 4 war dieDistraktionsbehandlung beidseitig aufgrund eines Teilrezi-divs nach Umstellungsosteotomie mit Erfolg durchgeführtworden. Es konnte eine deutliche Größenanpassung des Unterkiefers an den Oberkiefer mit stabilem Ergebnis unddamit eine Verbesserung des Profils erreicht werden.

Die Rolle des Kieferorthopäden in der Zusammenar-beit mit dem Kieferchirurgen bei der Anwendung der Dis-traktionsbehandlung liegt in der gemeinsamen, sehr präzi-sen Behandlungsplanung und in der Aufgabe, die Zahnbö-gen für die ein- oder beidseitige Distraktion vorzubereitenund Abweichungen der Kauebene rechtzeitig zu vermeidenoder zu reduzieren, indem das vertikale Wachstum gezieltgefördert wird und dentale Kompensationen und Zahneng-

is often used as a supplement to fixed appliance therapy tosupport distraction therapy. This seems to allow the newlygenerated bone to be formed before its consolidation, andthe direction of distraction to be guided to accomplish thedesired occlusal situation.

Mono- and bimaxillary osteotomies as well as augmen-tation using cartilage or bone grafts for correction of osseousdeficiencies of the midface and malformations of the noserepresented the gold standard for surgical facial reconstruc-tion of patients with mandibulofacial dysostosis prior to dis-traction osteogenesis becoming established in oral and max-illofacial surgery. This concept was employed exclusively inpatients 1 and 3. Patient 1, who was suffering from a unilat-eral malformation, had to undergo repeated mandibularaugmentations and finally a bimaxillary displacementosteotomy at the age of 26 years due to facial asymmetry anda deviation of the occlusal plane resulting from hypoplasia ofthe mandibular ramus. In addition to mandibular advance-ment following sagittal splitting of the ramus, patient 3 toohad to undergo maxillary osteotomy in the LeFort I plane foradjustment of the deviating occlusal plane.

Partial resorption of bone grafts is a common problemafter augmentation, often making repeat surgery necessaryand unfortunately necessitating in patients 1 and 3 reaug-mentation of the mandible and of the zygomatic arch respec-tively.

Due to insufficiency of the velar muscles and of theaccompanying soft tissue, the speech quality of patient 4remained inadequate despite velopharyngoplasty and subse-quent speech therapy. Nevertheless, although the temporo-mandibular joints were dysplastic, mouth-opening exercisesled to acceptable mouth opening without further measuresbeing needed.

The otoplasty of patient 2 is now being prepared. The physical and mental development was normal in all fourpatients. Compliance by both patients and parents was excel-lent. Systematic time scheduling with hospitalization beingconfined largely to vacation periods allowed school and uni-versity education to be continued without delay. Three of thefour presented patients have now successfully completedtheir degree courses, while the fourth completed an appren-ticeship after graduating from junior high school.

ConclusionsThe presented final outcomes after challenging treatmentillustrate that excellent rehabilitation outcomes can beachieved by interdisciplinary cooperation even in cases ofsevere congenital mandibulofacial dysostosis. Today, themethod of choice among the spectrum available for suchpatients is distraction osteogenesis, which can be used foreffective treatment exerting a favorable influence on growthdevelopment at an early stage. Experience gained by themedical team in treating such patients, improved intubationmethods and, last but not least, technical refinement of the

Distraktionsosteogenesen erfordern eine intensive undoft auch lange kieferorthopädische Nachbetreuung, um diekorrekte Einordnung der Zähne und das kontrollierteSchließen des seitlich offenen Bisses zu Ende zu führen. In-termaxilläre Gummizüge gleichzeitig oder anschließend andie Distraktion sind häufig zusätzlich angewendete Behand-lungsmittel zur Multibracketapparatur. Damit scheint esmöglich zu sein, den neu gebildeten Knochen vor seinerKonsolidierung zu formen und die Richtung der Distraktionzu korrigieren, um die gewünschte Okklusion zu erreichen.

Chirurgische Eingriffe zur Rekonstruktion des Gesich-tes durch mono- oder bimaxilläre Operationen und mithilfevon Augmentationen von Knorpel- oder Knochentransplan-taten zum Ausgleich der knöchernen Defizite im Mittelge-sicht sowie zur Korrektur von Nasendeformitäten waren vorEinführung der Distraktionstechnik das Grundkonzept derKieferchirurgen bei mandibulofazialen Dysostosen. Sie fan-den auch ausschließlich bei Patient 1 und 3 Anwendung. Pa-tient 1 mit einer einseitigen Fehlbildung benötigte wegen derGesichtsasymmetrie und einer schiefen Okklusionsebene alsFolge des hypoplastischen Unterkieferastes mehrere Unter-kiefer-Aufbauplastiken und schließlich eine bimaxilläreUmstellungsosteotomie, die im Alter von 26 Jahren statt-fand. Auch Patient 3 wurde wegen einer gekippten Okklusi-onsebene, zusätzlich zur Vorverlagerung des Unterkiefersdurch sagittale Osteotomie, im Oberkiefer in der LeFort-I-Ebene zu ihrer Nivellierung operiert.

Die partielle Resorption von augmentierten Knochenist eine bekannte Schwierigkeit, die oft Wiederholungspla-stiken zur Folge hat und leider auch bei den Patienten 1 und3 zu nochmaliger Unterkiefer- bzw. Jochbogen-Aufbaupla-stik führte.

Bei Patient 4 konnte wegen der insuffizienten Velum-muskulatur und des generell zu geringen Weichteilangebotesim Velumbereich die Sprache nach Velopharyngoplastik undanschließender Übungsbehandlung nicht mehr ausreichendkorrigiert werden. Trotz der Dysplasie der Kiefergelenkekonnte dagegen eine akzeptable Mundöffnungsgröße aus-schließlich durch Mundöffnungsübungen erreicht werden.

Die noch ausstehende Ohraufbauplastik bei Patient 2 ist in Vorbereitung.

Die körperliche und geistige Entwicklung war bei allenvier Patienten normal. Die Kooperation, auch mit den El-tern, war ausgezeichnet. Durch sinnvolle zeitliche Planungunter Ausnutzung der Ferien für Krankenhausaufenthaltekonnten Verzögerungen in der schulischen und univer-sitären Ausbildung vermieden werden. Drei dieser Patien-

SchlussfolgerungenDie hier gezeigten Endzustände nach aufwendiger Thera-pie können als Beispiel dafür dienen, dass auch bei schwer-wiegenden angeborenen mandibulofazialen Fehlbildungendurch interdisziplinäre Zusammenarbeit sehr gute Rehabi-litationsergebnisse zu erzielen sind. Den wichtigsten Platzunter den heute zur Verfügung stehenden Behandlungs-möglichkeiten bei diesen Patienten nimmt die Distrakti-onsosteogenese ein, die für eine effektive Behandlung mitfrühzeitiger günstiger Wachstumsbeeinflussung eingesetztwerden kann. Erfahrungen des Behandlungsteams mit sol-chen Patienten, eine verfeinerte Intubationstechnik sowietechnische Verbesserungen des Fixateurs zur Präzisierungder Distraktionsrichtung werden den operativen Behand-lungsaufwand straffen können. Die vor- und nachberei-tende kieferorthopädische Behandlung bleibt aufwendigund verlangt ein hohes Maß an fachlichem und apparati-vem Einsatz.
