Specific instructions on what needs to be filled into each section is provided here. Getting Started: Use a Google Chrome web browser (you can download a free copy here). DO NOT use the browser’s [BACK] button once you are entering Annual Report data. Maximize your browser wider for best results. The left navigation numbers indicate your progress. Once a section turns green it will be saved if you want to resume data entry later. Please read the Terms of Use. Main Menu: Low Gross Exemption Fee Request – Click this button if you will be filing for a Low Gross Exemption Fee request. (MRRC-4L). Multiple Site Single Fee Request - Click this button if you will be filing for a Multiple Site Single Fee request. (MRRC-4M). Standard Annual Report – Click this button to fill out the Standard Annual Report form (MRRC-2). First Data – Make a Payment – Click this button to make an online EFT payment.

Low Gross Exemption Fee Request – Click this button if you will be filing for a Low Gross Exemption Fee request. (MRRC-4L). Multiple Site Single Fee Request - Click this button if you will be filing for a Multiple Site Single Fee request. (MRRC-4M). Standard Annual Report – Click this button to fill out the Standard Annual Report form (MRRC-2). First Data – Make a Payment – Click this button to make an online EFT payment.

1. Enter your Mine ID. 2. Enter the User Code (provided in courtesy letter from DMR) 3. Check the box after reading the Terms of Use 4. Click the button to open up the Low Gross Exemption Request Form (MRRC-4L)

1. Enter the Annual Gross Income. 2. Upload Income Tax Return or CPA report – REDACT sensitive information before upload. 3. Enter Reclamation Plan Date. 4. Enter Financial Assurance Date Approved by Lead Agency.

A message indicating successful submittal should be displayed followed by the need to enter data in a Standard Annual Report form.

Standard Annual Report Site Login:
1. Enter your Mine ID. 2. Enter the User Code (provided in courtesy letter from DMR) 3. Check the box after reading the Terms of Use 4. Click the button to open up the Standard Annual Report form (MRRC-2)

Section 2 Enter the Mine ID, primary commodity and amount of production for every Mine that is included in this Multiple Site request. Must enter more than mine.

Section 3 Enter the name of the requestor and title.
Please complete a Mine Annual Report (Section 1-17) for every Mine ID that is part of the Multi-Site request. See guide for filling out a Standard Mine Annual Report.

Continue filling out - to Section 17. NOTE: Reporting fee is prorated per mine depending on number of Mines for the Multi-Site request. Example: If 2 mines are part of Multi-Site request reporting fee per Mine is $2656.00 ($5312 divided by 2)
