THE OCCLUSION, THE TMJ AND ORTHODONTIC TREATMENTOrthodontic Treatment Mechanics Cases

Prof. Dr. Richard P. McLAUGHLINDr. Richard McLaughlin completed his orthodontic training at the University ofSouthern California in 1976. Since then he has been in the full time practice oforthodontics in San Diego, California. He has lectured extensively in the UnitedStates as well as internationally. He is a member of the Pacific Coast Societyof Orthodontists, the American Association of Orthodontists, and a Diplomateof the American Board of Orthodontics. He is the Component Director of theSouthern California Component of the Edward H. Angle Society of Orthodontistsand Past President of the National Angle Society. He is the recipient of the2009 American Board of Orthodontics Dale Wade Award as well as the 2010Pacific Coast Society of Orthodontists Award of Merit. In addition, Dr. McLaughlinis a clinical professor at the University of Southern California, Department ofOrthodontics in Los Angeles, California, and an associate professor at SaintLouis University, Department of Orthodontics. He has written more than 30journal articles and co-authored 5 textbooks.

C orrection of a malocclusion to a position in whichthe condyles are in the correct position can belikened to the proper construction of the foun-

dation of a house. Without it, the house is subject to futureinstability, as is the malocclusion treated to the incorrectcondyle position. This course presents a comprehensivereview of the management of orthodontic patients withtemporo-mandibular disorders, as well as preventive

considerations. A historical review is presented, in whichthe concept of ideal occlusion is discussed, as well as itsrelationship to temporo-mandibular disorders. A protocalfor diagnosis and treatment planning, as well as splinttherapy is discussed. This is followed by a working clas-sification of TMJ disorders, and numerous case examplesdescribing the management of each category within theclassification.

n Historical ReviewPertinent aspects of the literature will be reviewed con-cerning occlusion and the temporo-mandibular joint. Adetailed discussion of static (centric) occlusion and func-tional occlusion will be presented. The controversialsubject of centric relation will be reviewed. During thislecture a classification concerning the skeletal and dentalcomponents of TMJ dysfunction will be presented.n Diagnosis and Treatment PlanningThe diagnostic process for the TMD patient isreviewed. The benefits and use of the articulator willbe reviewed. A three phase treatment planningprocess is then presented. n Splint TherapyA discussion of splint therapy will be presented as itrelates to management prior to orthodontic treatment,as well as to management during orthodontics.n Molar Fulcruming ProblemsCases that present with average to high mandibular

plane angles frequently show a molar fulcrummingproblem. Several of these cases will be presentedand the mechanics of their treatment will be described.n Lack of Vertical Posterior SupportCases that demonstrate a collapsed vertical dimensionwill be shown and methods of treatment of thesecases will be described.n Management of Anterior Slides and Anterior PosturingA number of cases in this category will be presentedwith comments concerning the use and misuse offunctional appliances.n Correction and Avoidance of Distal DisplacemenDistal displacement of the condyle is frequently seen inpatients with TMD. This subject is discussed and a numberof cases within this category will also be presented.n Management of Lateral Skids and True SkeletalDeficienciesDiscussion and case presentations concerning thesetwo categories will be presented.

He received DDS and PhD degree from Graduateschool, College of Dentistry, Yonsei University. He completed the orthodontics specialtytraining in Yonsei University. He was a visiting scholar at the Departmen of Biochemistry,University of Pennsylvania School of Dental Medicine in 2002-2004 and at theDivision of Plastic Surgery, the Children's Hospital of Philadelphia in 2010-2011. Hewas an adjunct professor at the Department of Orthodontics, Temple University, andat the University of Pennsylvania (2010-2011). He has contributed many book chapterson biomechanics of miniscrew-driven orthodontics, non-extraction treatment in adults,up-to-date lingual orthodontic mechanics and surgery-first approached using TADs.He is the first who demonstrated the miniscrew-assisted rapid palatal expander (MARPE)for adults in AJO-DO, which was cited by many other authors. He has published many

articles and case reports regarding the treatment of non-eruption, and total arch movement for hyperdivergent facein orthodontic journals including two cover issues in AJO-DO. His fields in research include clinical biomechanicsregarding TADs application and the suture and bone responses to orthodontic stimulus. He has served as a reviewerin major orthodontic journals, including AJO-DO, Angle Orthodontists and European Journal of Orthodontics. COURSE PROGRAMIn spite of the growing demand for the lingual orthodontic treatment among adult patients, biomechanicallimitations in three-dimensional control of tooth movement and difficulties in the access to the lingual side stillexist. In this course, an up-to-date lingual mechanics via combined use of virtual setup models, orthodonticminiscrews and self-ligating brackets(SLB)will be extensively demonstrated in the following aspects.

1. Simplicity in the diagnosisn Quick and easy diagnosis using three-dimensional template analysisn Visualization of treatment goal and midline-based disgnosis

2. Simplicity in the bracket bonding and alignmentn Application of CRC tray for lingual SLBn Readily available rebonding systemn Quick and easy leveling & alignment using universal leveling wire

3. Simplicity in the anchorage preparationn Symmetric/asymmetric anchorage using miniscrewsn Total arch movement in lingual orthodontics

4. Simplicity in the torque controln Arbitrary torque control using single wiren Troubleshooting-loss of torque during treatment

The Individua concept represents a digitallyplanned tray for high precision indirect bonding. The brackets are virtually placed in the optimalbonding position on the scanned model of thepatient and then transferred on the malocclusionwith the help of the Individua tray. The biggestinnovation comes from the revolutionary usedmaterial, with soft elastic properties that allowsprecise capture of the bracket’s form and posi-tioning, while preventing the tray from tearingduring the bonding process. Due to the featuresof the tray bonding in under 4 minutes per archis possible, resulting in a more comfortable bond-ing session for the medic, as well as for thepatient.

Lingual retainer produced withCAD/CAM technology for a perfectadaptation to the individual shapes ofthe teeth and a considerably reducedsize, that allows insertion also on theupper arch. The final format of the Mem-otain retainer is obtained by laser-cuttinga sheet of Nitinol, according to a digi-tally established design. Due to the pseudoelastic properties ofthe material used and the productionmethod that does not involve bending, the retainer does not have any predeter-mined weakening points.
